Survey Terms & Conditions

1. About this survey

• You are invited to take part in a Lenstore customer survey. Your feedback helps us improve our services and overall experience.
• Participation is voluntary.

2. Promoter

• The promoter is Lenstore, operated by EssilorLuxottica.

3. Eligibility

• Open to residents of the United Kingdom aged 18 years or over.
• Participants must complete the survey and provide a valid email address to enter the prize draw.
• Only one entry per person is permitted.
• Employees of the promoter and their immediate families are not eligible.

4. How to participate

• Participation involves completing an online survey hosted on SurveyMonkey.
• You must provide accurate and complete information.
• Entries that are incomplete, inaccurate or duplicated may be disqualified.

5. Entry period

• The survey and prize draw open on 16/04/2026 and close on 26/04/2026.
• Entries received outside of this period will not be included.

6. Prize draw

• All participants who complete the survey and submit their email address will be entered into the prize draw.
• One winner will receive either:
  • one £100 Amazon voucher, or
  • two £50 Amazon vouchers
• The winner will be selected at random from all eligible entries.
• The odds of winning depend on the number of entries received.

7. Winner notification

• The draw will take place on 27/04/2026.
• The winner will be contacted by email within 7 days of the draw.
• The winner must respond within 14 days to claim the prize.
• If no response is received, the promoter reserves the right to select an alternative winner.

8. Prize conditions

• The prize is non-transferable and cannot be exchanged for cash.
• The promoter reserves the right to substitute the prize with one of equal or greater value if necessary.
• Amazon voucher use is subject to Amazon's own terms and conditions.
• This promotion is not sponsored, endorsed or administered by, or associated with Amazon.

9. Data protection

• Personal data will be collected and processed in accordance with applicable data protection laws including GDPR.
• Data will be used to administer the survey, run the prize draw and improve Lenstore services.
• Data is collected via SurveyMonkey and handled securely.

10. General

• The promoter reserves the right to amend or withdraw this promotion where necessary.
• Any participant found to be in breach of these terms may be disqualified.
• By participating, you agree to be bound by these terms and conditions.
